PPO members

Occasionally, you may need to obtain services from a provider that has not contracted with the PPO network. These provider types may include, but are not limited to:

  • Ambulatory surgery facilities
  • Home health care providers
  • Hospice providers
  • Outpatient mental health care facilities
  • Substance abuse facilities
  • Skilled nursing facilities

When such care is needed, select your actual plan type and the search will return participating Traditional providers whose services are covered under your PPO plan. When you receive services from the Traditional providers displayed, you will be subject to in-network benefits.
