Get records about a living member
(when you are a third party and not a member on the contract)
For example, you:
- have power-of-attorney
- are an attorney
- are a life insurance carrier
- are an in-state or out-of state agency
Do you need PHI information about a specific date of service or range of dates, medical procedure or therapy for a member?
Our Customer Service representatives can help you with this request. The member must first submit the Authorization for Use and Disclosure of Protected Health Information (PDF) authorizing us to disclose their PHI to you.
You may then call the Customer Service phone number on the back of the member's ID card. If you don't have the ID card, call the Blue Cross Blue Shield of Michigan corporate operator at 313-225-9000. You'll need to provide the member's enrollee ID number.
Or, if you are the member's personal representative, you must submit documentation showing you are the personal representative. Mail your documentation to:

If you need a HIPAA designated record set, complete the Request for Access to Designated Protected Health Information Records (PDF). The member must submit the Authorization for Use and Disclosure of Protected Health Information (PDF) authorizing us to disclose their PHI to you.
Or, if you are the member's personal representative, submit documentation showing you are the personal representative. Mail your documentation to:
