Reporting suspected fraud
You can report suspected wrongdoing confidentially in any of the following ways:
Telephone
8:30 a.m. to 4:30 p.m. Eastern time Monday through Friday.
- Call our confidential Anti-Fraud Hot Line at 1-800-482-3787. After-hours calls will be recorded. An anti-fraud specialist will return the call the next business day to collect further information.
- To report Medicare-related fraud, call our confidential, dedicated Medicare Anti-Fraud Hot Line at 1-888-650-8136. TTY users should call 1-800-588-2711.

Complete and submit our online form. If you are concerned about remaining anonymous, please be aware that the e-mail form is neither secure nor encrypted. All information we receive is strictly confidential.
Mail or fax
Download, print and return the anti-fraud complaint report (PDF). Please include your contract number and the date of service the complaint is about. The mailing address and fax number prints near the bottom of the form.
Did you know?
The Blues' two anti-fraud hot lines receive more than 2,000 calls per month.
Approximately 30 percent of these calls result in an investigation by our fraud unit. The remainder of the inquiries is referred to other areas of the company for administrative follow-up.
