Preadmission notification

For continuity of care we encourage the following facilities to notify us when admitting a Medicare Plus Blue PFFS^SM member:

• Hospitals, including those with swing beds
• Skilled nursing facilities
• Rehabilitation facilities, both free-standing facilities and units within hospitals
• Long-term care hospitals
• Mental health facilities, both free-standing facilities and units within hospitals

However, you are not required to request or receive prior authorization to admit Medicare Plus Blue PFFS members.

 

Acute care hospitals with web-DENIS access should use it for preadmission notification. All other health care should call the preadmission notification line.

 

You will need to have the following information:

• Health care facility name and type of facility
• Member's name, sex and contract number
• Attending physician's name and phone number
• Estimated length of stay, admitting diagnosis and any planned procedures
