Complete the Automatic Payment Enrollment Form (PDF) and mail it to us. If you prefer, fax your completed enrollment form to us at 313-983-2605.

Internet banking

Ask your bank about making Blue Cross Blue Shield of Michigan one of your "payees" when paying bills online. This way, you can make one-time payments to BCBSM when it's convenient for you.
