Do you know when you're due for your next checkup? Do you remember what tests your doctor performed during your last visit? Have you forgotten what your doctor said about the symptoms you're experiencing?
Use our Personal Medical Care Journal to help keep track of medical care for both you and your children. Make as many copies as you need and use it for medical services, dental care or ongoing therapy.
Here's how to use the Journal:
- Write down your symptoms before seeing a doctor. Record any questions because patients often forget when they're in the examination room.
- Bring the journal with you and use it to guide the discussion with the doctor. Be sure to write down what the doctor says at that time.
- Keep the journal in a safe place.
