Description: In this initiative, Physician Organizations will work with Practice Units to adopt an "extended access" model of care delivery which includes 24-hour patient access to clinical decision-maker; after-hours access to non-ED urgent care with link to a patient's PCMH; advanced access; and a multi-lingual approach to care.
