Description: In this initiative, Physician Organizations will increase adherence to evidence-based medicine, reduce gaps in care, and improve upon overall patient outcomes. Physician Organizations will implement effective systems of care designed to support outreach to populations of patients with identified primary and secondary prevention needs, as well as chronic illness management needs.
