Blue Cross Blue Shield of Michigan offers two care management programs. Both programs help you focus on your health so you can feel your best.
If you have a chronic condition, you may qualify for one or both of these care management programs.
If you qualify for the program, a care team, led by a nurse, will work with you to create a personal care plan. The care plan will help you better take care of your health.

The care team will communicate with you through phone calls, email and messaging via our mobile app. You may get text reminders and educational materials from your care team as part of the program.
The program includes a free mobile app, powered by Wellframe. The app will help you stay on track with managing your health, communicate with your care team and set medication reminders.
Your care team will regularly check in to help you understand how to manage your chronic condition, coordinate care after a hospital discharge and answer your questions. They can also help you find services in your area that meet your specific needs.

Landmark Health delivers in-home medical services to members who qualify with multiple chronic conditions. The goal of the program is to help you stay healthy at home and avoid unnecessary emergency room visits and hospital stays.
A key element of the program is Landmark’s coordination with your existing primary care doctor and specialists.
This program is available for eligible Medicare Plus BlueSM or BCN AdvantageSM members in Michigan's Lower Peninsula.
