After you call and schedule, a doctor or nurse practitioner will come to your home. You should plan on a 60-minute visit. You should have your medications handy, write down questions ahead of time, and have a friend or relative with you during the visit.
After the visit, the doctor or nurse practitioner will write up a summary of the visit and your health status. This will be shared with both you and your primary care physician as a record of the In-Home Visit.
Your wellness exam includes:
- A review of your medical history
- Checking your blood pressure, heart rate and other vital signs
- Talking about the medications you're currently taking
- Answering your health questions
