When you enroll in ConnectedCare: Centrus Health, you receive all your medical care from Centrus Health providers and facilities located in five counties in the Kansas City area: Clay (MO), Jackson (MO), Platte (MO), Johnson (KS) and Wyandotte (KS).
If you have been enrolled in GM’s Basic medical plan option, you’re familiar with the terms in-network and out-of-network. ConnectedCare uses benefit levels:
- Level 1: When you use Centrus Health providers and facilities, your care is considered Level 1. Level 1 costs less than Level 2.
- Level 2: When you don’t use Centrus Health providers and facilities, this care is considered Level 2. Care from Level 2 providers and facilities costs more than Level 1.
Emergency care for qualified medical emergencies is covered at the Level 1 benefit, whether you’re in these five counties of Kansas City or traveling outside the area. Once discharged from the emergency department, all follow-up care should be received from ConnectedCare: Centrus Health providers.
If you and your family receive non-emergency medical services from providers in these five counties that aren’t in Centrus Health, but are in the BlueSelect Plus network, services will be covered at the Level 2 benefit, with higher out-of-pocket costs. If you receive non‐emergency medical services from providers not in Centrus Health or the BlueSelect Plus network, these services won’t be covered.
Non‐emergency services received from providers in the following Missouri counties aren’t covered: Andrew, Atchison, Bates, Benton, Buchanan, Caldwell, Carroll, Cass, Clinton, Daviess, Dekalb, Gentry, Grundy, Harrison, Henry, Holt, Johnson, Lafayette, Livingston, Mercer, Nodaway, Pettis, Ray, Saline, St. Clair, Vernon, Worth.
For services received from providers in all other counties in Missouri and Kansas (not listed above), covered non-emergency services are covered at Level 1 when you use a PPO provider.
