What’s a Health Assessment and How Do I Take It?

The Blue Cross Blue Shield of Michigan online health assessment is a health survey that helps you get a picture of your current health and your health risks. You'll learn your strengths as well as areas for improvement and get personalized recommendations to help you tackle your well-being goals. This resource is available to you and your eligible family members 18 and older at no added cost.

Why should I take the health assessment?

  • It's fast. Mobile-friendly and interactive. It takes about 15 minutes to complete. 
  • It’s effective. It asks a variety of basic questions about your diet, exercise, sleep, medical history and lifestyle factors to give you a holistic view of your current health and a personalized plan for better health.
  • It’s personal. After you complete your health assessment, you’ll receive everything you need to help you improve your health:
    • A health score based on an analysis of your modifiable health risks
    • A list of your risk areas
    • Information about steps you can take to improve your health

Please note that not all our plans include a health assessment through Blue Cross Well-Being. If your plan doesn’t, you won’t see a link to well-being when you log in to your account.

How to take a health assessment

Step 1: Log in to your account

Your health assessment is only available through your online account with us. You can log in on our website or use the Blue Cross Blue Shield of Michigan app.

After you log in, go to the Programs & Services tab, then select Blue Cross Well-Being under Quick Links.

Don’t have an online account? You can register for one on our website or the mobile app. Just keep in mind, each adult taking a health assessment will need their own account.

Step 2: Take the Health Assessment

From the Blue Cross Well-Being site, select the Health tab, then Surveys. 

Login tips

  • Make sure you can access your online account before your health assessment is due. You need to know your username and password. 
  • If you can’t remember your username or password, you can recover them online. Have your Blue Cross ID card and the answers to your security questions handy.

The information in your health assessment is considered confidential and will be disclosed only as permitted by federal and state privacy laws.
