Do I Need to File a 1095-B Tax Form?

A 1095 tax form provides proof to the IRS that you and your family had health insurance during the year. The type of form you could receive will depend on the type of plan you purchased.

In the past, if you bought your health plan directly through Blue Cross Blue Shield of Michigan or through certain employers, we automatically mailed you the 1095-B form. The government has made changes to some of the forms that you need to file with your taxes. You don’t need to file a 1095-B form, so we’ll no longer mail one automatically.

You can view your 1095-B form by logging in to your online account.

If you’d still like a copy, you can request it and we’ll provide it within 30 days.

  • Send your request to Blue Cross Blue Shield of Michigan, 600 E. Lafayette Blvd., Detroit, MI 48226.
  • Call the phone number on the back of your subscriber ID card.
