Provider Data Attestations

Per the Consolidated Appropriations Act, providers must update and attest to the accuracy of their provider directory data every 90 days — even if no changes are needed.

In addition, professional practitioners are required to attest to all other data elements every 120 days. This includes elements related to credentialing and licensing.

Failure to complete required attestations will result in being removed from our provider directory and may affect our ability to process claims on your behalf.

How to attest

Professional practitioners

Professional practitioners can complete attestations by logging in to the CAQH Provider Data Portal.
